A new personalised care plan service has been launched to help care home residents and their families choose what care they wish to receive in an emergency.
The Coordinate My Care (CMC) service provides a personalised care plan, including the patient’s diagnosis, medications, wishes and preferences, what to do in an emergency and their resuscitation status.
Care homes are being asked to start the plan for their residents by completing an online questionnaire by going to https://www.coordinatemycare.co.uk/for-patients/mycmc/ and clicking ‘Start myCMC Plan’.
The completed questionnaires goes to the resident’s GP who will then share the plan with all urgent services, including out of hours GPs, the ambulance service and the emergency departments so that they are familiar with a person’s history and needs when called upon.
CMC will be sending all London care home managers posters to put up in their care homes and post cards that can be given to the residents and their families. Further information can be obtained at www.coordinatemycare.co.uk.
