After a flurry of last-minute nominations, we have decided to extend the deadline for the 2019 Leaders in Care Awards, giving you an extra two weeks to get your entries in.
Award hopefuls must now enter by Friday, August 16 by downloading the nomination forms below and emailing them firstname.lastname@example.org.
Companies are invited to nominate themselves by filling in an official nomination form, or they are free to nominate any other businesses they feel deserve recognition.
There are two nomination forms – one for the care home provider categories and one for the supplier categories. Both are incredibly simple to fill in and can be downloaded below.
The categories are listed below and detailed descriptions can be found by clicking on each nomination form.

Once the nomination process has closed, shortlisted entries will be voted for online by the industry to determine the winners.
The winners will be announced at a gala ceremony taking place on the first night of the Care Show, Wednesday 9 October, at the Hilton Birmingham Metropole.
The evening will start at 7pm with a drinks reception followed by a three-course dinner and the presentation of awards to the country’s leading providers and suppliers across residential and domiciliary care.
